Two ways: the La Costeña flight from Managua (1 h 30) or the long overland route: bus to El Rama or Bluefields, speedboat, and then the boat to Corn Island, which only sails on certain days.

La Costeña flies from Managua airport via Bluefields for about US$100–200 per leg depending on the season. Book ahead around holidays.
Night express bus from Mayoreo to El Rama (6 h) and a speedboat (panga) down the Escondido river to Bluefields (2 h). Combined night departures reach Bluefields mid-morning.
Passenger boat from the Bluefields dock, 5–6 hours, about C$280–320. It usually sails Wednesdays and Saturdays at 9:00 AM; tickets are sold only at the port office, so arrive the day before.
Public panga at 10:00 AM and 4:30 PM (back at 6:30 AM and 1:30 PM), about US$10 per person and a 30–40 minute, sometimes bumpy, ride.
Approximate cost
Flight US$100–200 per leg. Overland: C$250–300 + C$250–300 + C$280–320 + US$10, about 24 hours in total.
Managua to the Corn Islands without flying
They took the 11:15 a.m. bus from Managua's Costa Caribe terminal to Bluefields: C$320, an assigned seat and about 7 hours with two short breaks. Then the 9 a.m. ferry to Big Corn (C$255, 6 hours) was a perfectly smooth crossing on a glassy sea, with drinks sold aboard; the panga on to Little Corn is C$360. All for far less than flights at around US$200 each way.

Granada to Little Corn by land and sea
They went from Granada to Little Corn without flying: a bus to El Rama, a speedboat to Bluefields, the Río Escondido ferry to Big Corn and a panga, about US$40 in all against US$164 for return flights. They call it quite an adventure and actually quite fun. They planned 3 or 4 days on Little Corn, but with the diving, snorkelling and fresh fruit they stayed about 10.
